Grade Exclusion Form

If you are eligible to exclude grades, print the Grade Exclusion Form, enter the required information, obtain the required signatures and turn it in to the Records Section, Office of the Registrar, General Services Complex, 750 Agronomy Road, Suite 1501, by the published official deadlines.
