FERPA Forms and Resources

Below is a list of FERPA forms and resources that faculty and staff may find helpful.

The staff members of the Office of the Registrar are available to participate in a FERPA question and answer session for any department or college faculty meeting. To make arrangements, please call the Office of the Registrar at 979-845-1711 or submit a request to ferpa@tamu.edu.

Permission to Release Education Record Information
This document is to be utilized when a student would like to grant permission to a TAMU employee to disclose their educational records/data to a third-party. The Office of the Registrar recommends TAMU employees utilize this form so the student can indicate the specific education records/data allowable for release, to whom the records can be released, for what purpose, and provide an expiration date for release consent.  This form is not intended to replace either the Certification of Dependency process or Howdy self-service access, but should be utilized to document specific instances of student education record release. University offices should maintain this completed document in their records, following their retention policy.


Consent to Release Education Records/Data for Letter of Recommendation for Graduate/Professional Schools
This document is to be utilized when a student would like to grant permission to a particular entity to discuss educational records/data when writing a letter of recommendation for graduate/professional school. The Office of the Registrar recommends the student indicate the specific education records/data allowable for release, and to whom the records can be released. Faculty/staff should ensure a signed release is obtained from students for any letter of recommendation when personally identifiable information is to be included. University offices should maintain this completed document in their records, following their retention policy.
 
 
Consent to Release Education Records/Data for Letter of Recommendation for Employment
This document is to be utilized when a student would like to grant permission to a particular entity to discuss educational records/data when writing a letter of recommendation for employment. The Office of the Registrar recommends the student indicate the specific education records/data allowable for release, and to whom the records can be released. Faculty/staff should ensure a signed release is obtained from students for any letter of recommendation when personally identifiable information is to be included. University offices should maintain this completed document in their records, following their retention policy.


FERPA Statement of Responsibility/Non-Disclosure Agreement for Employees
This document is to be utilized to document employee understanding of FERPA and their responsibilities for student education records. The Office of the Registrar recommends to utilize this form during the onboarding of new employees, including student workers. University offices should maintain this completed document in the employee records, following their retention policy.


FERPA Non-Disclosure Volunteer Form
This document is to be utilized to document individuals serving in a Texas A&M University volunteer capacity as a school official according to FERPA, and indicates that the volunteer understands FERPA and their responsibilities for student education records.  The Office of the Registrar recommends individuals (e.g., community members, students) complete this document prior to serving in a volunteer role that includes access to Texas A&M University student education records. University offices should maintain this completed document in their records, following their retention policy.


FERPA Addendum
This document is to be utilized during the contract phase for third party vendors who deal with student data to ensure that vendors are in compliance with FERPA. The document should be appropriately personalized with PROVIDER information and necessary contact information for PROVIDER and Texas A&M University contact.
