Deans and Department Heads are provided a Signature Card Authority Reauthorization Form of their designees and their approval access prior to the start of the upcoming academic year.
The deadline each year to submit new signature cards to the Office of the Registrar will be between mid to late August and will be announced when reauthorization cards are sent to the deans and departments heads. Submitting a Department or Dean Signature Card Form will ensure designees have the appropriate access effective through August of the following year. Otherwise, designee access may expire as of August 31st of the current year.
Once completed, these should be returned to the Office of the Registrar as follows:

Please remember to keep a copy of your submitted documents for your records.
Signature cards will become effective as soon as they are received. These signature cards are utilized by the Student Services (Records and Registration) and Degree Audit sections to process academic action requests submitted to our office.
